Provider Failure
Record the failure and re-evaluate the route. If an alternative execution provider is available, it must pass the full validation pipeline before execution.
Bridge Failure
Record state, notify where required, and require route revalidation before retry.
Stale Data
Reject the trigger.
Simulation Failure
Reject execution.
Permission Failure
Reject execution.
Monitoring Failure
Mark affected sessions DEGRADED.
No failure path may silently expand authority.
